Medical claims desk

How is denied-claim work judged?

Did you choose the right action for each claim the first time, and keep every insurer deadline alive? Do your appeals and calls cite the right policy with the right evidence? And is the book left in an order the next day can work from?
  • Each line restates a rule in the Medical claims desk's grading design. Most parts are scored by fixed rules against the case; the written appeal and the account notes are read against a checklist.
  • 4 graded areas, each one below

Graded area 1

The action you chose for each claim

Reads the insurer's payment notice correctly, line by line, before asking the AI helper.
Finds the real cause of the denial, and opens the fact that proves it before submitting.
Takes the best path on the first submission. Where the best path is to accept the loss, accepts it the first time.
Appealing what cannot be won: a true duplicate, or a Medicare denial for filing too late.
Writing off a claim that could have been won.
Reaching the right answer only after several rejected submissions.

Graded area 2

The deadlines you kept

Makes every submission inside the insurer's limit.
Answers each deadline warning on the day it appears.
Leaves no claim open at the end of the shift without a next-action date.
Letting a right to appeal lapse.

Graded area 3

Your calls and appeals

An appeal that names the claim, the date of service and the denial by its code, and asks for an amount that matches the lines appealed.
Names the policy that decides it by document, section and version, and quotes its wording exactly.
Sends the specific records that win, matched to the criterion each proves, and nothing irrelevant.
Follows up waiting claims on a steady rhythm, and keeps the reference number of every call.
Sending the whole medical chart where specific records were asked for.
Citing an out-of-date version of the policy.
Acting on a call that has no reference number.

Graded area 4

What you left for the next day

Works the book in order of value and urgency, with a reason written wherever you depart from it.
Puts any claim close to its deadline first.
Gives every claim you park a reason and a date to come back to it.
Parking a claim with no reason or return date.

Questions

Asked about this desk

Does the desk check for compliance?

Yes. Changing a diagnosis, a visit level or a modifier that the medical notes do not support fails the case, and sending more records than were asked for counts against you.

How does the AI helper fit in?

It suggests a reading and a path, and it is sometimes wrong. Accepting a correct suggestion counts for you; accepting a flawed one, or overruling a correct one, counts against you.
